What are Community Health Workers?

A Community Health Worker (CHW) is a frontline public health worker who is a trusted member of the community and has a strong understanding of the community served, healthcare system, and resources available.

Why are CHWs Needed?

CHWs work closely with clients to address non-medical needs and other concerns that may impact an individual’s ability to manage their health.

CHW services are free and confidential for patients and community members.

What Services do CHWs Provide?

CHW’s serve as a link between health/social services and the community. Their work facilitates access to services that improve the overall health and wellness of the communities they serve. The health/social services they support may include the following:

  • Food
  • Language
  • Transportation
  • Insurance
  • Housing
  • Finding Primary and Preventative Care

How do CHW’s communicate with patients?

Once an individual is enrolled as a CHW client, Community Health Workers communicate with their patients by phone, patient portal (MyChart), in-person visits, in-home visits, and virtual visits. 

How can working with a CHW help me?

Working with a CHW can help you feel more confident managing your health. You will receive easy to understand health information and support setting goals for your health. The CHW will help you connect to resources in your community.
